Privacy Policy

Last updated: June 29, 2026. This policy covers the LiveWell website, contact forms, scheduling inquiries, communications, and consumer health data. Protected health information created during care is also covered by our Notice of Privacy Practices.

Overview

LiveWell Psychiatry & Men's Health is a small psychiatry and men's health clinic serving Oregon and Washington. We collect the minimum information we need to respond, schedule, coordinate care, run the clinic, and keep the site secure.

Information we collect

When you contact us, we may collect your name, phone number, email address, state, age range, patient status, insurance preference, preferred contact method, reason for reaching out, and any message you choose to send. If you use the patient portal, scheduling tools, payment tools, or texting, those systems may collect additional account, appointment, billing, and communication details.

Like most websites, we also collect limited technical information such as browser type, device type, pages viewed, referral source, and basic security logs. We use privacy-respecting analytics and we block third-party ad trackers on this health site.

Clinic safety and CCTV

Our physical office uses CCTV audio and video recording for safety and security. Recordings are not used for advertising or sold. If a recording includes health information, we handle it under the Notice of Privacy Practices and applicable law.

Consumer Health Data

LiveWell maintains a separate Consumer Health Data Privacy Policy for Washington, Oregon, and similar consumer health data laws. That page explains the categories of consumer health data we collect, where it comes from, how we use it, when we share it, and how to make a request or appeal.

Protected health information

Protected health information is handled under HIPAA and our Notice of Privacy Practices. Don't use ordinary email for urgent clinical details. Current patients should use the secure portal when a message belongs in the chart.

Texting and mobile information

If you give us your mobile number, we use it for clinic communication such as appointment reminders, scheduling, account notices, and replies to your inquiry. We don't sell mobile information and we don't share mobile opt-in data with third parties for marketing. See our Text Message Terms.

Your choices

You can ask for access, correction, deletion, or other privacy help by emailing info@livewellwith.us or calling (503) 303-3330. If we deny a request and the law gives you an appeal right, you can appeal by replying to our decision or emailing the same address with "Privacy Appeal" in the subject line.

Changes

We may update this policy as the clinic, law, or website changes. The latest version lives on this page.
