Consumer Health Data Privacy Policy
Last updated: July 2, 2026. This is LiveWell Psychiatry & Men's Health's consumer health data privacy policy. It applies where Washington, Oregon, or similar consumer health data laws apply. Protected health information created during clinical care is also covered by our Notice of Privacy Practices.
Categories of consumer health data
The categories of consumer health data we may collect include information you provide about mental health symptoms, sleep, focus, medication questions, men's health concerns, sexual health concerns, weight, hormones, insurance, appointments, care preferences, and communications with the clinic. If you're a patient, we may also receive clinical, lab, pharmacy, referral, payment, and portal information.
Sources of consumer health data
Sources may include you, your parent or guardian if applicable, our contact and scheduling forms, the patient portal, clinicians and staff, labs, pharmacies, insurers, referral sources, payment processors, and communication vendors used to operate the clinic.
Why we collect and use consumer health data
We collect and use consumer health data to respond to inquiries, schedule visits, verify insurance, provide care, coordinate with other providers, handle billing, run secure communications, document clinical work, comply with law, prevent fraud or misuse, and improve clinic operations.

We may share only the categories needed for the purpose: contact details, appointment information, insurance and billing information, clinical information, lab or pharmacy information, referral information, portal messages, and records needed for legal or regulatory duties.

Third parties and affiliates that may receive information include our clinicians and staff, our electronic health record and patient portal vendor, scheduling and communication vendors, labs, pharmacies, insurers, payment processors, professional advisors, regulators, and service providers that help us operate the clinic. livewellwith.us and psychiatry.help are operated by LiveWell, LLC. The education site is separate from clinical scheduling.
No sale of consumer health data
We don't sell consumer health data. We don't share consumer health data for third-party advertising.
Your consumer health data rights
You can ask us to confirm whether we collect your consumer health data, access it, correct it, withdraw consent where consent is the basis for processing, delete consumer health data where the law requires deletion, or appeal a decision. Clinical records may also be governed by HIPAA, state medical-record laws, and provider documentation duties, so some requests may be handled under those rules instead of ordinary website privacy rules.
