Acceptance

By using this website, you agree to these terms. If you don't agree, don't use the site.

Informational only, not medical advice

The content on this site is general information. It isn't medical advice, diagnosis, treatment, or a substitute for a visit with a qualified clinician. Browsing the site, reading an article, or submitting a form doesn't create a provider-patient relationship.

In an emergency

This website and our contact forms are not emergency services. If you're in immediate danger or having a medical or mental-health emergency, call 911. For the Suicide & Crisis Lifeline, call or text 988.

Clinical communication

Current patients should use the secure patient portal for clinical questions that belong in the chart. General contact forms, email, and text messages are useful for scheduling and logistics, but they're not a 24/7 line and they're not a crisis line.

Licensure and service area

We provide clinical care only where we're licensed and where the care is clinically appropriate. We're licensed in Oregon and Washington. If you're outside those states, the website can still accept a message, but we may not be able to see you clinically.

Tools and screeners

Calculators, symptom checks, screeners, and other tools on this site are for orientation and conversation prep. They aren't a diagnosis, treatment plan, or guarantee that a medication, lab, or visit will fit.

Appointments, insurance, and payment

Appointment availability, insurance participation, benefits, prior authorization, and out-of-pocket estimates can change. Coverage estimates aren't a guarantee of benefits or payment, and final responsibility can depend on your plan.

Information you submit

Use your own accurate contact information. Don't submit someone else's private information unless you have authority to do that for them.

Privacy and patient notices

Our Privacy Policy explains website, communication, and consumer health data practices. Our Notice of Privacy Practices explains how protected health information may be used and disclosed. Our Text Message Terms explain SMS communication.

Third-party links and tools

The site may link to our patient portal, app stores, social profiles, payment tools, education articles, maps, or other third-party services. Those services have their own terms and privacy practices. We link to them because they're useful, not because we control everything they do.

Education site relationship

livewellwith.us is the clinic and scheduling site. psychiatry.help is LiveWell's education and editorial site. Education articles are for learning and don't replace clinical care.

Intellectual property

The LiveWell name, logo, design, and site content belong to LiveWell, LLC or are used with permission unless otherwise noted. Don't copy or reuse them as your own.

Changes

We may update these terms. Continued use of the site means you accept the current version.
